Electronic Consent Forms:  Detailed information and guide was emailed to all parents/guardians Sept. 16  

For all Grade 8 students: Electronic Consent Forms will be done online through a District Forms secure site: (https://forms.sd41.bc.ca/Forms/sdconsentac .) 

For all Grade 9 – 12 students:  If you have NOT previously done the consent for Cariboo Hill Secondary School, then you must complete the Electronic Consent Forms.  If consent was given for Cariboo Hill Secondary previously, the form does not need to be done again. 

If you have more than one child at Cariboo Hill Secondary School, you must fill out one consent form for each child.  We kindly request that you fill out these permission forms as soon as possible. 

Emergency Release Form:  An Emergency Release Form (PDF fillable) was emailed to all parents/guardians

We kindly request that you fill out this form as soon as possible, save it and then attach it to an email and send to Cariboo.Info@burnabyschools.ca or you may print it and have your child bring it to the office.  Paper copies of this Emergency Release Form are also available at the office. 

We will keep it on file for the remainder of the school year.  All students Grades 8 – 12 must have a form completed each year.
